Liquor Self- Exclusion Form and Termination Form
Bankstown Sports Club offers Liquor Self-Exclusion assistance. If you need assistance with any liquor problem please contact the duty manager for assistance 02 9722 9888. A photo will be required to complete the exclusion forms.
